Collaboration and community partnership in healthcare is life-changing.
Waterloo Regional Health Network (WRHN) has had a long-standing partnership with The University of Waterloo School of Pharmacy, working together to bridge education and real-world practice to enhance patient care.
Many dialysis patients at WRHN are managing chronic conditions alongside end-stage renal disease, and often require dozens of pills each day. Hemodialysis pharmacist Keith Miller, a UWaterloo alum, works closely with care teams to adjust, streamline, and safely reduce unnecessary medications for patients like these.
“You’re not just making a recommendation and hoping it works. You’re able to follow up, adjust and really see the impact of your care,” Miller tells the University of Waterloo.
Patients in this program are incredibly resilient; they’re managing a chronic condition full-time on top of their other responsibilities.
This approach to care can help patients avoid unnecessary hospital visits, prevent medication-related complications and achieve better health outcomes.
